Fees and Payment Options

Welcome to the Birth and Women's Health Center.   It is our intention to provide you with a safe and loving haven for your prenatal care and birth.   In an effort to give you the best possible experience and care, our staff is available to explain our financial policies and agreements, and to assist you in understanding your insurance benefits.   However, please be aware that it is ultimately your responsibility to determine your coverage.

If you do not have insurance, or if it does not provide you with 100% coverage, please meet with our OB Case Manager to discuss payment plan options.   If you do not have insurance, you may be eligible for Arizona's program for maternity care: Baby Arizona (SOBRA).   We can provide you assistance with the application .   A Reduced Fee Program may be an option for people who are uninsured and cannot afford care.   Costs are based on income of the entire family and an application process is required.

** Please note: No discounts can be applied for patients who are covered by any insurance plan. **

Self Pay: Billing for the self pay (uninsured) patient is separated in two parts, “Prenatal Care” and “Facility Fee”.  The total combined cost is $5,450.    Please be aware that there may be additional costs depending on procedures, lab tests, or transfer of care to another provider.  The fees must be paid by the 34th week of gestation.  The fees may be prorated in to monthly payments.

**The prices stated in this publication are subject to change and are not a guarantee.  Your fees will be based on your family’s income and all patients must have a written contract to receive services.

Insurance :   Please provide us with a completed and signed patient registration form, along with your insurance card.   We will bill your insurance company at the end of your pregnancy.   Any fees not covered by your insurance company will be your responsibility.   If your insurance coverage changes, please inform the front office immediately.

Financial Agreement :  For patients covered by medical insurance you are responsible to pay your co-pay at your first visit.  We recommend that you verify your covered benefits and be aware of deductibles and co-insurance.  For Self Pay patients payments are to be made monthly, with total payment due four weeks prior to your due date.   If you are unable to meet your payment schedule, please talk with the OB Case Manager.
